ACCEPTANCE TO SERVICE POLICY
Effective 01/01/25
Acceptance to service is contingent upon assessing the anticipated needs of the prospective patient, evaluating the case load and mix of current patients, assessing staffing levels and availability, and ensuring that the staff possess the necessary skills and competencies to deliver high-quality care. The Agency maintains policies for the receipt, processing and the evaluation of persons referred for services.
Eligibility Criteria for Acceptance to Service. The Agency will ensure patients meet basic eligibility criteria such as:

Physician’s, or appropriate practitioner, order and medical necessity

Patient’s residence is in the geographic service area

Patient or legal guardian’s consent

Insurance/payment coverage
Available Public Information.
List of Services Offered: Intermittent Nursing and Rehabilitation services which are PT, OT, SLP and MSW. including specific skill levels such as specialized wound care, psychiatric services, palliative care, infusion therapy, diabetic care, catheterization, gastronomy feeding, and ileostomy/colostomy care.
Geographic Service Areas include the following: Harris, Colorado, Brazoria, Waller, Austin, Wharton, Fort Bend, Houston, Polk, Trinity, Brazos, Grimes, Chambers, Galveston, Montgomery, Liberty, Walker, Washington, Matagorda, Angelina, Nacogdoches, San Jacinto, Tyler, Fayette, and Madison.
Potential Service Limitations: this may include service duration, geographic service area, and specialty services. At this time, we do not accept the following:

Specialty Services: patients on a ventilator and psych patients.

Service Frequency: patients are seen intermittently and not daily.
This information will be updated as frequently as the services are changed but no less often than annually.
